Bilateral Coronary Artery-Pulmonary Artery Fistulas
نویسنده
چکیده
CORONARY ARTERY FISTULAS, precapillary communications between a coronary artery and a cardiac chamber or vessel, have been described as the most common hemodynamically significant congenital coronary anomaly.' In a review of 21 original cases and 342 previous cases, Levin et al.' reported that coronary fistulas originated slightly more often from the right than from the left coronary artery, but that bilateral fistulas those that originate from both coronary arteries accounted for only 5% (19 of 363) of the total. These bilateral fistulas are unique in their tendency to terminate in the pulmonary artery. More than half of the bilateral, but only 17% of the unilateral fistulas, terminated in this manner. In this report, we review the nine previously described cases of bilateral coronary artery-pulmonary artery fistulas and present five additional cases studied at our institutions between December 1976 and March 1981. The anatomic uniqueness of these fistulas and some considerations in their management are discussed.

Bilateral coronary artery fistulas with the coronary artery stenosis are rare. In this case, we successfully performed closure of coronary artery fistulas with coronary artery bypass grafting. Furthermore, we were able to measure the flow in the coronary artery fistulas using transit-time flow measurement.

Introduction Coronary artery fistulas (CAF) are congenital or acquired formations between the coronary arteries and cardiac chambers or other vascular structures such as the vena cava, pulmonary artery, or veins [1]. Most of these are found incidentally during angiography [2]. Fistulas account for half of all coronary anomalies and are present in 0.002% of the general population. Coronary to pu...

A 58-year-old woman was admitted to our department for evaluating an abnormal blood flow in the main pulmonary artery by transthoracic echocardiography. Coronary angiography revealed the presence of fistulas originating from the left anterior descending artery and also the right coronary artery to the main pulmonary artery. Under cardiopulmonary bypass, both fistulas were ligated near the origi...

A coronary artery fistula is a direct communication between a coronary artery and one of the cardiac chambers or vessels around the heart. These fistulas are usually diagnosed by coronary arteriography. Clinical presentations are variable depending on the type of fistula, shunt volume, site of the shunt, and presence of other cardiac conditions. Bilateral coronary fistulas between coronary arte...
